Welcome to a training platform designed exclusively for Western Australia’s Home Stretch providers.
Developed as part of the Home Stretch WA Community of Practice (CoP) a team of lived experience consultants worked together with the CoP backbone team to design, develop and deliver the first core training packages for the Home Stretch WA roll out through a series of in – person Training Packages from July 2022- December 2024.
To support the sustainability of training, induction, and model integrity, the Home Stretch WA CoP developed Online Learning Modules for service providers. The modules offer a general induction into the Home Stretch WA Service Model, aligned with the Model Integrity Framework. They incorporate previous practice hour recordings, facilitated by the CoP and Service Providers, training materials from the statewide rollout and feature insights from Transition Coaches , Lived Experience Consultants and young people participating in the Home Stretch WA between July 2022 and December 2024.
This training is designed to be complemented by reflective practice and hands-on practical learning through service delivery by Home Stretch WA Service Providers.
These modules serve as an online archived resource and are no longer updated as of July 2025.
The Home Stretch WA Community of Practice is a self-sustaining network that fosters continuous improvement and collaboration. Policy and Practice Working Groups bring together Service Providers and the Department of Communities to regularly review and update policies, practice guidelines and relevant documentation.
Practice functions are maintained by Home Stretch WA Service Providers through peer-led reflective forums, including practice hours, Coordinator meetings, and ACCO-led practice sessions.

This training guide is to support Home Stretch WA Transition Coaches & Coordinators in their induction to the Model. It consists of video recorded sessions and presentation slides of previous training. It is designed to be followed from the first Practice Area – Home Stretch 101 through to Staying On and additional topics.
The learning is targeted at frontline staff who are part of a provider team, supporting young people leaving out-of-home-care. Other supporting team members may also benefit from participating in the online learning.

Training can be completed individually, in 1 – 1 sessions between the Coordinator and Transition Coach or as a team where the Coordinator can facilitate using the training modules or slides.
It is recommended to allocate minimum 3 hours per week for onboarding training within the first month to ensure Essential Trainings have been covered.
