Community of Practice Knowledge Hub- Previous Training & Resources
The Community of Practice (CoP) Knowledge hub was developed to support the roll out of the Home Stretch WA model from July 2022 – December 2024 by providing access to practice tools and resources. It was historically facilitated by Anglicare WA in consultation with Home Stretch WA (CoP) members. Service providers are encouraged to adapt resources to their local context and use the Home Stretch WA Model Integrity Framework to guide model integrity.
The Community of Practice (CoP) Knowledge Hub is an archive created for the following
- Developed in consultation with Home Stretch WA providers
- Ensure alignment with the model’s core principles according to the Home Stretch WA Model Integrity Framework
- ‘Allow providers to adapt resources
The Home Stretch WA Community of Practice (CoP) was created as a central resource to support service providers in delivering the Home Stretch WA Model between July 2022 and December 2024. It provided access to practice tools and resources, developed in consultation with CoP members, to help implement the model effectively. The CoP encouraged collaboration, shared learning, and adaptation of resources to suit different local contexts, with the aim of enhancing the consistency, quality, and effectiveness of service delivery.
Originally facilitated by Anglicare WA, the CoP was guided by the Home Stretch WA Model Integrity Framework to ensure alignment with the model’s core principles.
