Understanding Transition Coaching
A consistent, persistent and skilled support worker who provides flexible, one-to-one support to a young person, focused on coaching towards interdependence.
Through the Transition Coach relationship, a young person becomes empowered, builds their self-reliance, skills, capacity and connection to community, culture and country.Young people valued consistent communication via SMS and digital platforms, with coaches maintaining contact even when the young person wasn’t responsive.
- Coaches should work closely with young people’s existing support networks, especially during transition phases and to ensure continuity beyond age 21.
- Young people want access to a broader support team when their coach is unavailable—especially for those with disabilities, young parents, or higher needs.
- Stable relationships with coaches are essential, even when young people move regions, with flexible digital and local collaboration ensuring ongoing support.
- Building a strong relationship takes time; coaches must use a youth-focused, persistent approach tailored to young people with complex support histories.
- Aboriginal young people must be supported by culturally aware coaches, with Aboriginal Community Controlled Organisations playing a central role in delivery.
A Transition Coach is a dedicated support worker who helps young people navigate the shift from the child protection system into independent adulthood, typically up to age 21 and beyond. Their role is to stay consistently connected with the young person—often through digital means—and provide tailored, responsive support across housing, education, wellbeing, and life skills. Transition Coaches also work collaboratively with the young person’s existing support networks, including family, carers, and community services, ensuring a stable and culturally appropriate foundation for independence.
