Home Stretch WA provides young people leaving out-of-home-care with the option to continue to receive support up until the
age of 21 years.
The Home Stretch WA Model Integrity Framework is designed as a
tool to support service providers to establish a high-quality Home Stretch WA service.
It is a service model co-designed by and with young people.
It communicates a shared understanding of what Home Stretch WA service must offer in WA to be considered a Home Stretch WA service. The framework also describes how Home Stretch WA should be delivered based on
the evidence base and insights shared by members of the Home Stretch WA – Community of Practice.
